In order to bill for the 11730 nail avulsion, is it required to be numb?

Can someone please explain to me the difference between the terms "simple" and "complicated" when researching the procedures.
I have a physician who consistently states a "slant back" procedure was done in office but does not state whether patient was numb/not numb , just trying to find some sources to refer him back to.

It states that if there was no anesthesia used, the provider must state why there was a lack of anesthesia. Typically the provider will document if the procedure was complicated. Don't ever assume, and if it isn't clear, then send a query. It's always better to ask than be wrong and have denials.
