I am having a debate with another coder regarding time documentation for Smoking Cessation. Does the doctor have to specify the amount of minutes spent or is this documentation acceptable:

"SMOKING CESSATION COUNSELING, 3-10 MINUTE (CPT code 99406) During a face-to-face interaction with patient, I asked about his tobacco use; assessed his readiness for smoking cessation, I advised patient to quit smoking in a clear personalized way, discused the benefits of smoking cessation and gave him the phone number for the national smoking quit line."
 
This is what our providers use:


Preventive Medicine

Counseling:
Smoking Cessation
Patient counselled on the dangers of tobacco use and urged to quit. 03/23/2016
Pt also counselled about the effects smoking has on chronic pain 03/23/2016
Time spent counselling patient on smoking cessation 3 to 10 mins

If the time spent is greater than 10 minutes and 99407 is used the providers with document that 10 mins or greater
