We are having some conflict with the documentation for the social history for newborns. What would be an acceptable social history for a newborn? Physicians are saying things like "Family lives in Atlanta." Please provide examples.

Some that come to mind are:

Living arrangements - does child live with parents? Guardians?
Parents marital status
What type of house - year built, exposure to lead paint, mold?
Do they live in rural or underserved area without access to emergency healthcare?
Use of drugs or alcohol by caregivers/parents
Exposure to smoking
Any domestic violence in the home
Parental employment/education
Maternity/Paternity leave
Patient in day care?
Sleeping arrangements
Any support system for parents to help care for child?
Other children in the home
Household pets
