Hi friends,

My physician is asking where to write the birth history of baby in e/m history section.
can it be noted under social history.
he is a ethical man so,he want to document all the birth history of baby.

Medical history

Birth history for infants is their past medical history.

Social history would be something like - baby lives at home with parents and two older siblings.
