The Scenario:
Room and board is being billed on a UB04 with Revenue Codes for their treatment, with the "Provider" being the Facility, and the Attending being the Medical director. The services took place IN the facility. Box 32 - Facility Location - should that be the Treatment center info, or the Physician Group info? Box 33, Billing Provider - is that the Physician's group, or the actual doctor/professional in the group who performed the svc, CPT code 90818?
Block 18 - Hospitalization dates - this should be entered if they are in an in-patient setting. Does a Residential Treatment center (POS code 55) count as "inpatient" enough to have to fill that out?
