Is speech therapy usually covered under the patient' s medical plan? Does anyone have experience billing for speech therapy? Is this based on time?

Every policy is different. You should be able to call the insurance company to confirm the patient has benefits. Most insurance companies require prior authorization for therapy services. Then, once authorized, they limit the number of visits/treatments.

There is modifier with speech therapy of GN, add minutes on record , pre approval required, add referring doctor name and want date if first time vs previous date of speech therapy. Usually related to strork or autistic or neurology diseases.
