Can someone please help with the correct codes. I didn't copy entire report but there are findings for all selective catheterizations. I would very much appreciate help on this.

A 5 F catheter was then introduced into the patient and used to
selectively catheterize the following vessels using fluoroscopic roadmap
guidance:

Right subclavian artery
Right thyrocervical trunk
Right vertebral artery
Right common carotid artery
Left common carotid artery
Left vertebral artery
Left thyrocervical trunk
Left subclavian artery
Right T5 radicular artery
Left T5 radicular artery
Left supreme intercostal artery

I would code it with the following:

36226-50 (bilateral vertebral, following the hierarchy rules the bilateral subclavian selection is included in 36226)
36222-50 or 36223-50 (bilateral common carotid, code choice depends on if the imaging was of the extracranial or intracranial carotid circulation)
36218x3, 75774x2, 75705x1 (bilateral thyrocervial and left supreme intercostal)
36215x2, 75705x2 (bilateral T5)

You'll have some bundling issues but the 59 mod would be warranted. Hope that helps :)
