Can someone help me with segments and levels for arthrodesis proc code 22612. The op note states posterior spinal fusion L3-sacrum. Do I code the primary 22612 L3-L4 or is this just L3 and the addons would be 22614 L4, 22614 L5, 22612 S1 or 22614 L4-L5 and 22614 L5-S1?? This code is throwing me off because it says "single level" but I am not sure if I am supposed to be combining

The span T12-L4 contains 5 vertebral segments (T12, L1, L2, L3, L4) BUT only 4 vertebral interspaces (T12-L1, L1-L2, L2-L3, L3-L4)

If I understand your scenario, you have L3-L4, L4-L5, L5-S1 This equals to:
