I'm still pretty new to spine coding, but I think that the anterior interbody lumbar fusions are coded w/ 22558 x2 , the posterior lateral fusion as 22612, and the cages as add on codes 22853 or 22854 (ask your doctor for clarification), and check out the spinal instrumentation section for the percutaneous instrumentation which starts at +22840 in the ICD manual. I hope this helps! :)
 
My thoughts are:
22558
22585 (second level)
22853 X 2 (2 cages)
Don't see any anterior instrumentation in your procedure so we won't code for that

22612. (first level of posterior lateral fusion)
22614 X 3 (next 3 levels of fusion)
22842 (covers all of the posterior instrumentation segments includes rod and screws)
