Please tell me if I have coded this correctly and if I am missing any codes: T-Lif at L4-5 with implant 22633,22840,22853 and new code 63052 and 15777. Then he is going to do posterior lateral fusion at L5-S1 with verteboc which is an allograft not hardware. so I was going to use 22612, 63047-59-51. Am I missing anything?
 
Without seeing the note:
For the L5-S1, I would use 22614. This add-on code can be used with 22633 for a second level without interbody fusion. If he does a decompression at that level, 63047-59 (no 51). 63047 bundles w/22633. Don't forget your bone graft codes. What is the 15777 for?
 
Tough to say without the op note. Correct advice above, you can't code 22612 with 22633 per NCCI edits and guidelines. Read the NCCI manual Chapter IV musculoskeletal guidelines.

Is 63047 for a lami at 5-1?
and no hardware except at the TLIF level? So, 22840?

Yes, why 15777? For the allograft?
 
Has anyone coded 22633 and 63052 for a Posterior Lumbar Interbody Fusion (PLIF)? I know 63047 is included in 22633 but can you code 22633 and 63052 together and get reimbused for both?

15777 for biologic implant barrier, keeps scar tissue from adhereing to the nerves, applied during spine surgery. I think it is called AmnioFix barrier.
