Can someone clarify stable/improving vs worsening for me? Below is how I interpret these scenarios but now I'm not so sure. I'd appreciate any feedback.

1) If a patient has an established problem that has improved from last time (back pain, uncontrolled diabetes) but is not resolved or at goal, does the provider get 1 point or 2 points under the Number of Diags/Mgmt Options portion of the MDM? I'd give 1 point since it's improving even though it's not at goal.

2) If there's no change, is that 1 point or 2 points? I'd give 2 points because even though it's not worsening, it's not improving either.
