I am working on wound care records to validate the diagnosis for Risk Adjustment, and the physician documents the following:

Wound on the right plantar foot measures 0.5 cm x 0.4 cm x 0.2 cm.
There is minimal drainage. Mild erythema is noted. It is stage II.

IMPRESSION: Ulcer stage II.


There is no indication if this is a pressure/ decub ulcer, but he stages the ulcer. Would this (stage II) validate the use of 707.07 and 707.22?
