I work for a behavioral health clinic. Deal mainly with LPC's. Does anyone have a sample chart or know of a site I can go to. Currently the organization I work for uses different formats for each payor. We are looking to streamline and standardize all charts. We primarily deal with DSHS, Texas Medicaid and several private insurance. Looking for something to the hightest standard to meet all requriements. Hope someone can point me in the right direction.
