Many of the Medicare LCD's no longer mention that the % of stenosis has to be documented, only that it is be a "hemodynamically significant" stenosis.
Does anyone have any additional guidance/guidelines etc that support the % stenosis being documented or support other terms (ie: severe stenosis, significant stenosis etc) or is everyone still encouraging the % to be documented?
