Do you still need to document a chief complaint with new 2021 E/M rules?
 
Nothing has changed other than History and Exam are no longer a required key component in choosing the E/M. They still need to be documented, including the CC, they are just not used to determine E/M levels now. This allows the doctor to document only what is medically necessary for the History and Exam. Imagine that.
