I need tips/tricks to get my HCC accuracy better. I do have a couple of questions on diabetes codes. I have seached the ICD 10 and other resources. My question is when would E11.8 be used?(If a pt has HTN and Hyperlipidemia?) When would E11.40 apply as apposed to E11.42? (If documentation only says "Neuuropathy") And lastly, any advise as to how can I increase my accuracy?

I Would like to hear about this also. I am having a hard time with my HCC accuracy and need to be at a 95% or higher. I also have trouble with E11.40 vs E11.42. I am working with inhome and virtual visits.
