My provider is billing: 61512,62140,61781,69990. (for the cranioplasty code he did dictate in his note that it was less than 5 cm)
After reviewing the op note I believe coding should be: 61519,62140-59,61781,69990-59
Which is correct? I'd appreciate any help.
