Things like councilor to client ratio would be part of the state regulations. In Texas that would be the Department of State Health Services (DSHS) They have a MHSA division that would would license treatment centers and addiction professionals. They would be the best resource to make sure you are doing the treatment correctly. The State Medicaid rules should align closely with the state regulations. As per billing the groups, the LCD might be a good source as for the requirements to bill the code, but I would bet there is not definition of the group size which is why the state regulatory agency is probably a better resource for your question.
