Hi all!
I am assisting a new pediatrician office with their forms and was wondering if there is a standard form or consent that needs to be filled out by guardian or parent of child if they wish to allow another family member to accompany child to their pediatric visit? I would appreciate any help. Thank you.
 
Yes I would make sure that you have at least a signed letter from a legal guardian that gives permission for the person to bring the child as well as have HIPAA information. Most offices create their own standard form for this, or they require a letter from each parent/guardian in such cases.
You do not mention the age of the child..... keep in mind that in each state, laws differ on some certain services that children can be seen WITHOUT a parent or guardian at all,,, mostly they usually include sexual issues or mental health visits/evals. -- check your states regulations on that for specifics.
