There was a chart found in the ICD-9 for 2010, listed in the coding guidelines that had it as a third DX code placement. The books no longer contain the chart. Hope that helps :)
 
The chart never had a column for 3rd listed , there was first- only, second only or either first or second. I do believe this code is acceptable as either first or secondary.
