Hi,
Having an issue with Supartz billing. We have reasearched and can not find the correct way to document the information on the claim form.

We were told that we need the NCD, units of measurement qualifier and the NDC quantity (number of NDC unites administered) listed out in the description.

The supartz is 25mg / 2.5 ml syringe . the code is J7321 Hyaluronan or derivative, Hyalgan, Supartz or Visco-3 for intra-articular injection, per dose

So would it be N4enterNDC#here UN 1 or N4enterNDC#here ML 1 or N4enterNDC#here ML 25mg/2.5ML ?

from one paper I found the listed are the preferred NDC units of measure and their descriptions

UN (unit) powder for injection (needs to be reconstituted
ML (milliliter) liquid, solution or suspension
GR (gram) ointments, creams, inhalers or bulk powder in a jar

I am soooo confused!
