I DO NOT charge out surgeries without an op report.

Can someone please advise me where I can find something in writing that states this, I have a doc who does not "understand" .

I cannot seem to find the source of this information (common sense?)

How about the golden rule of the coding world---not documented, not done! If you can't substantiate a service with documentation to prove that it happened, how can you charge for a surgical procedure (or anything else)? If your practice was audited and no supporting documentation existed, you'd be sending payment back.
