When reporting DX either chronic conditions or not do you all look for some type of supporting plan that the DX was evaluated/treated?
I'm used to doing this for HCC coding but I'm new to wound care coding and I don't see alot of supporting plans for every dx the podiatrists listed.
For example:
Diabetic foot ulcer and only talks about the ulcer, would you report DM without notation if the DM is stable, controlled on metformin etc...????
I know the guidelines state to report all conditions that coexist and I know the Diabetes can impact the wound. So I am going back and forth on this! Help!!
