Hi,
Can anyone help me with billing, 28810 then a few days later billing 11042 ? The 11042 keeps getting denied as being in the 90 day care inclusion.

Unplanned return to OR?. Staged or related procedure during the post op period, unrelated procedure in the post op period? What modifier did you use. What are the circumstances of the second procedure?

Procedure was planned and done as inpatient hospital, we didn't use a modifier because it was a different dos.

Lack of use of a modifier is why its being denied as global. You always need a modifier if doing a surgical procedure during the global period of another surgery.
