I need some help on this surgery. Pt came in for incisional hernia repair, while going in provider runs into a lot and I mean a lot of adhesions (22 mod) on proc. So while taking down the adhesions he lacerates the bowel x4. I cant bill for that am I correct? Plus he also nicked the rectum. Like I said the adhesions were that bad.
My question: Since he lacerated the bowel trying to go into the abdomen can I bill for the repairs?

No if he caused the complications can't bill for them. However, if he dictated time on the lysis of adhesions you could put a 22 modifier and sent a note with op explaining why.
