We have recently added a surgeon to our medical staff so I am still learning on the modifier side of things. Can someone help me as far as a "59" modifier goes. Does it need to go on the least expensive procedure or the most expensive?

It goes on the minor procedure. It is the modifier of last resort. Here is a decent decision tree, in this article.

I would also encourage you to review the X modifiers. They are often a better fit to justify the unbundling of services.

And only use it if there is an NCCI edit conflict. If not, you don't need it.
