My dr did a 27130 LT in the am, in the pm of the same day he took the patient back in and did a 27134 78 52. UHC received both claims on the same day, but processed the 27134 and now wants a modifier on the 27130. Any ideas out there.
 
I'd submit the medical records, the second op report should clearly state that the patient had surgery earlier in the day and this is a second procedure.
 
The OP notes were submitted for both procedures with detailed notes of the timeline. UHC is stating they must have a modifier on the 27130 to process it.
