we have had problems with payors denying surgical implants rev code 278 as global even when we show the invoice and that our physician paid for them separately.
we normally bill them with the surgical procedures on the UB. anyone have any suggestions, should we bill them separately???
 
You should contact the payer directly. There is no set answer. They are bundled into ASC charges for many payers, In many cases, only specific pass through implants are paid separately. The rev code is fairly meaningless. What HCPCS code are you submitting with the REV? They payer may be able to answer if they will accept it on the 1500 form as a pro fee.
