Dr did mass excision on foot,
came back to do skin graft, anthem paid skin graft with dx S913.02 but denied the 11042 and 11045 stating invalid dx. What dx should i use for the debridement that was done to prepare the sight for the skin graft?

I think your issue to the CPT being billed. Look at the codes in the section 15002. These codes include preparing the site for a graft, which is what was done via debridement.
