Suture removal for new patient seen only by nurse

How would a nurse visit for a new patient in for only a suture removal placed at a different facility be billed. CPT 99201 has been deleted. Would you still use 99211 - an established nurse visit?
 
You cannot bill a new patient under incident-to. There has to be an established plan of care in place with the MD under whom the nurse visit is billed. The original facility is being paid for the reason for the suture placement, so that's where the patient should go. Otherwise, the new patient must see an MD or APP in your practice, establish a plan of care, and then the sutures can be removed.
