I am having difficulty getting paid for synagis. Typically, it is just a nurse visit so I am only listing 90378 & 96372 on the claim. Since the synagis is provided to our office 90378 carries a $0 charge and this line does not make it to the payer. Naturally they are denying the admin since they see no product. I was told by our software company that I would have to put a charge amount on a line for it to make it through the clearinghouse. Is placing a one cent charge on an item provided to our office legal?
 
If I remember correctly, I always billed 96372 with V04.82. If it was done during an exam, I would bill the appropriate E&M with diagnosis for the exam and 96372-59/V04.82.
