I work in skilled nursing facility and I was told R55 syncope we should not code for when billing private insurance companies and medicare. Should I code the therapy dx as primary?

Your primary DX should be the reason for the encounter or service. If the patient is having a therapy session, the therapy DX would be primary, followed by the DX(s) explaining the need for it.
