I am having a trouble sorting out what level of risk to select. If I have a new dx and it isn't a self limited dx but there is no rx given, would this qualify as a sf it lc? I have been assuming low.

It is a little hard to answer your question without looking at it but I would say that it could be an acute uncomplicated problem which would be a low and/or if OTC meds were given this would also be a low. Hope this helps
