My cardiologist has begun assisting on some TAVR procedures at the hospital. I have billed with the 62 or 66 modifier and I35.0 and Z00.6. These patients are NOT part of a clinical research study and therefore I have no 8 digit NCT number. I keep getting denials. What am I doing wrong??
