Hi, We're a teaching hospital and I'm working with our ER providers on their documentation. I recent reviewed a few notes that were transfer of care between attendings due to shift change. The first Attending added his attestation for the resident but I've notice that the second Attendings are not adding an attestation. I emailed the Attending and he states they have never done so before. I believe this is required since the attestation from the initial Attending is for any documentation written during that Attending's shift and since the Resident is still documenting on the second attending's note, he will need to attest to the resident's documentation. One key thing: Both Attendings/Residents notes are documented under one note. (They don't create separate notes.) Note basically states, transfer of care to Dr. Smith from Dr. Peter at 6:00am and the second attending starts his documentation. Any thoughts? And how would you response to your provider?
