In a teaching institution does the teaching physician attestation statement/ GC modifier still apply when the resident performs the history, exam and medical decision making, but the teaching physician documents the entire encounter? It is almost like the teaching physician is being a "scribe" just in the sense as they are documenting what the resident is doing.

This means there is no resident documentation for the teaching physician to attest to, but the resident is still participating in the management of the patient. I cannot find any documentation that touches on this subject. Any advise is much appreciated.
