Hello,
I am new to billing for teaching physicians. I am wondering if in this scenario if the delivery would have been considered done by the resident or the preceptor.

The note states "Dr (resident) was present the entire exam and I was present for the entire procedure, stepping in to deliver the shoulder".
Any help is appreciated.
