We have received a few denials related to bundling of the TEE for Mitral Valve Replacement from Coventry.
We Coded and sent records with the following codes:

00562 AA
93503 59
93312 26,59
93320 26,59
93325 26,59
36620

However, the TEE code 93312 is denying for bundling. Is this correct? Is this something we coded incorrectly or need to appeal? The anesthesiologist did the work, and the documents show that. However, they are still bundling the services.

Please help.
