Q3014 is the originating site code. Is this billable (and how) if the patient is in a hospital (not rural) and the physician is elsewhere?

Yes, this is billable by the facility where the patient is located. If the patient is in a hospital, it is billed as a line item on the hospital's UB-04 claim along with all of the other facility services provided during the encounter.
