IIs anyone else experiencing denied claims for procedure code 93798 from UHC? They’re denying it due to non-payable diagnosis codes, even though the claims were billed with the payable DX codes per NCD guidelines. After speaking with two different claims representatives, I received conflicting reasons for the denials. The first rep said it was due to an invalid modifier (95) and suggested we bill with GQ instead, while the second rep said the issue was with POS 10 being invalid and advised using POS 11 for office. This is the only insurance denying our claims with procedure code 93798, and I suspect they’ve implemented an algorithm specifically to deny these services. Any assistance would be appreciated.
