Do physician's need to include documentation in their progress notes that telehealth services were performed?

WIth any services billed to the third party payors, the addage is "if it isn't documented, it wasn't done". Dates and specific time frames are important.
 
Our physician that performs telehealth services states in the body of the HPI that the service was performed with the use of an RP-lite robot. This way we can attach the appropriate modifier (GT) to the claim. If the provider does not document the use of the robot, then we, as coders, have no idea if/when one was used and are unable to code the visit correctly.
