A doctor wants to start doing templates in our ASC on operative reports. Are they legal?. It seems that the operative report would say the same on most of the same procedures. Is this something that auditors look at -help :confused:,
 
Some of our doctors do them and I know they do them at the hospital as well. They just have "fill in the blanks" for each patient and can add anything else they need too as well. Most of the time they do have variations so they don't look like cookie cutter op notes.
