1. Placement of 13cm temporary hemodialysis catheter for acute kidney injury

Under US guidance, the right internal jugular vein was cannulated followed by a wire. A small incision was made. The tract was dilated and 13cm catheter was placed.
Both ports aspirated and flushed easily and were capped. The catheter was sutured in place and dressed.

I want to code 36556 but the ports are throwing me.

Also, do you code Z99.2 with acute kidney dx?

You are correct with the 36556. The catheter was two external "ports" to hook up to the dialysis machine, so don't worry about that. Do not code for u/s vascular access as there is no documentation of an imaged saved. For the ICD code, that is probably a "history code". You want a code for the N section of the ICD-10.
