Template for 98940-98943 Medicare

Hi

I am trying to find a good template to assist our chiropractor in improving his/her documentation for Medicare. If anyone has anything they would be willing to share I would be so very thankful.

Have you checked out your LCD from your MAC? It has very specific information on requirements. From there you may be able to create a template.

Check your local LCD. Also check for your EHR guidelines if that is what you are using. With the upcoming changes for 2017, you're going to want to know.
