Template for E&M coding in Oncology

Does anyone have a template that their oncology physicians use in seeing patients in the office? I am working with an oncology practice, and we have tried several templates, but have not found one that she is comfortable with.
We are using 97 guidelines, any help would be greatly appreciated.
