Good morning all,

Our company has acquired some pediatric practices and I was looking for a comprehensive form that I can introduce to them, for an E/M visit. I have come across some very good well child forms but not a really good "multi-system" age appropriate form. If anyone can point me in the right direction I would greatly appreciate it.

These are difficult to find, but a comprehensive Internal Medicine encounter form works well! You may have to modifiy it a bit, but much of what you need should be there. :)
