my doc started a port-a-cath insertion on left :

Initially the left chest was prepped and draped in standard fashion. Local anesthesia was injected below the clavicle. The subclavian vein was needle punctured. Multiple attempts were made to advance the J-guidewire through the subclavian vein but it met stiff resistance and would not advance. Contrast was injected through the cannulation needle and a venogram was performed demonstrating occlusion of the subclavian vein just beyond the needle access site most likely secondary to previously placed Port-a-Cath. Therefore the procedure was terminated from this side and proceeded on the right..........

op notes continues with successful insertion on right...is there additional codes i can use for the attempted left??? TIA
