If a provider is reviewing some diagnostic test result/findings with the patient at their follow up visit, doesn't the provider need to document the DOS of which diagnostic test they are reviewing?

II am getting mixed information, as I thought including the DOS ensures proper identification of the specific test being reviewed to make sure they are not speaking of the same one at every single visit.

I think you're overthinking this. Why would a provider discuss a test result from the past that is no longer relevant (e.g., a glucose test from three years ago?). I'm assuming this is for E/M level determination? It does not matter; the results of tests are still being discussed. Hope this is helpful.

Did you know there is a dx Z51.81, Z76.00 and Z71.2 related to meds management, new prescription given and testing results? You d probably want to use dx Z71.2 for last dx on claim if doc review lab results. I d use all these as last Z dx on claims per documentation. I agree with SWorld above if discussing lab test must be current.
