We have been billing 0249T (THD) and 45505 (Proctoplasty) together to most all insurances. Medicare and BCBSM/BCN do not pay for 0249T stating it is investigational. If a patient chooses to have the THD anyway they are signing an ABN and paying out of pocket. The problem that I am having is the hospital that we work out of will not bill for 45505 with 0249T stating it's inclusive. I have checked NCCI and several other resources trying to find if these codes are indeed bundled. All input is appreciated.
