what should be done if insurance is denying CPT 97112-GP as inclusive with other procedure 99245. Modifier GP signify 'service delivered under an outpatient physical therapy plan of care'.

physical therapist should not be using CPT 99245. What type of provider is providing the services.

You cannot use office visit codes for physical therapy.

If a PT seen pt 1st time, code is 97001-25. If an OT seen pt, code is 97003-25.

Re-evaluations for PT is 97002-25. OT is 97004-25
